Schedule:
4 days a week / 10 hour shifts 7:30am - 6pm
Position Overview:
We are currently seeking a highly Experienced Acute Care Inpatient Case Manager with strong discharge planning expertise to join our team. This role requires professionals who can step in with a minimal training, adapt quickly, and manage patients efficiently in a fast-paced Acute Care Environment.
Key Responsibilities:
Facilitation of precertification and payor authorization processes. Facilitation of the collaborative management of patient care across the continuum, intervening as necessary to remove barriers to timely and efficient care delivery and reimbursement. Application of process improvement methodologies in evaluating outcomes of care. Support and coaching of clinical documentation efforts and serving as a clinical resource for coders ensuring that documentation accurately reflects severity of illness and intensity of service.
Requirements:
Minimum 5 years acute Inpatient Case Management Experience Experience in acute discharge planning BSN Required BLS certification through AHA Previous experience using CarePort system.
