Trinity Health PACE Alexandria offers continuous, quality care for the elderly, enabling independent living at home. Seniors receive comprehensive care from interdisciplinary teams, fostering health improvement and community relationships. We serve with compassion and integrity, transforming lives within our communities. Position Purpose The Registered Nurse (RN) - Home Care is responsible for delivering skilled nursing care to PACE participants in their homes, ensuring comprehensive, coordinated, and compassionate care. This role collaborates closely with the interdisciplinary team to assess, plan, and implement care that supports participants' independence and overall well-being.
What You'll Do:
Provide skilled nursing care to participants primarily in their homes, ensuring safety, comfort, and quality outcomes Conduct comprehensive assessments and develop individualized care plans in collaboration with the interdisciplinary team Monitor participant health status and respond promptly to changes in condition Administer medications, treatments, and interventions as ordered Educate participants and caregivers on disease management, medications, and care plans Coordinate care with physicians, therapists, and other care team members Document care in accordance with regulatory and organizational standards Participate in interdisciplinary team meetings and contribute to care planning discussions Ensure timely communication of participant needs, concerns, and changes in condition
Minimum Qualifications:
Graduate of an accredited School of Nursing; Associate or Bachelor's degree required Current Registered Nurse (RN) license in the state of Louisiana Minimum of one (1) year of nursing experience required; experience in home care, geriatrics, or PACE preferred Valid driver's license with reliable transportation and a clean driving record Ability to work a flexible schedule, including occasional weekends
