A Nurse Case Manager applies nursing expertise to coordinate a patient's medical care and provides advocacy to help them get the best and most affordable care. Develop care plans including discharge planning, coordinate delivery among providers, and teach patients and families how to follow the plan. They may also help patients navigate insurance and other healthcare bureaucracies and access related social services. May work with patients who have chronic illnesses or other complex medical needs. Works at a hospital or other healthcare facility; may follow patients throughout the span of their treatment.

Job Description

Hospice RN Case Manager Location:
Lansing, Mason, Dewitt, and surrounding areas
Job Type:
[Full-Time] We're hiring a Registered Nurse (RN) Case Manager to provide skilled nursing care and manage patients at the end of life.
Key Responsibilities:
Complete patient assessments and develop plans of care Provide skilled nursing visits (wound care, meds, education, end of life care, etc.) Coordinate care with physicians and interdisciplinary team Supervises HHAs and maintains compliant practices Participates in on call rotation for holidays and when on-call RNs are off Document visits accurately and timely Participate in biweekly and PRN IDG meetings Must be okay with traveling throughout service area
Qualifications:
Active Michigan RN license 1+ year nursing experience (hospice) Strong communication and organization skills Valid driver's license and reliable transportation HCHB experience
