Under the direction of the Director of Nursing or Designee, the MDS Nurse will conduct and coordinate the development and completion of the resident assessment in accordance with the requirements of this state and the policies and goals of this facility. Know, understand, incorporate, and demonstrate the Mission, Vision, Values and Guiding Behaviors of Trinity Health and Trinity Senior Living Communities in behaviors, practices, and decisions. Consistently demonstrate (leads by example) Sanctuary Values to all internal and external customers (residents, visitors, volunteers, and associates.) Create and maintain an atmosphere of warmth, propagating a calm environment throughout the community Execute assignments in a culture that is shared and collaborative across all divisions within TSLC. Conduct and coordinate the development and completion of the resident assessment (MDS) in accordance with current rules, regulations, and guidelines that govern the resident assessment, including the implementation of RAPs/CAA and Triggers/CAT's. Conduct or coordinate the interviewing of each Resident for the Resident's assessment. Evaluate each Resident's condition and pertinent medical data to determine any need for special assess activities.
MINIMUM QUALIFACTIONS
Must possess, as a minimum, a Nursing Degree from an accredited college or university, or be a graduate of an approved RN program. Must possess a current, unencumbered, active license to practice as a Registered or Licensed Practical Nurse in this state. Must be knowledgeable of nursing and medical practices and procedures, as well as laws, regulations, and guidelines that pertain to long-term care. 2-3 years MDS experience.
