Job Summary Join our dynamic healthcare team as a Health Home Registered Nurse Case Manager, where you will play a vital role in coordinating comprehensive, patient-centered care for individuals with complex health needs. In this energetic and impactful role, you will assess patient conditions, develop personalized care plans, and collaborate with a multidisciplinary team to ensure optimal health outcomes. Duties Conduct thorough patient assessments, including vital signs, medical history, and physiological evaluations to determine individual needs. Develop and implement tailored care plans that incorporate discharge planning, post-acute care coordination, and home health services. Manage clinical case management activities by monitoring patient progress, adjusting interventions, and ensuring adherence to treatment protocols. Collaborate closely with healthcare professionals across hospital medicine, behavioral health, infectious disease care, and primary care to optimize patient outcomes. Utilize the Electronic Medical Record (EMR) system to document medical records accurately while maintaining strict compliance with HIPAA regulations. Facilitate communication with patients and their families to provide education on medical terminology, medication management, and health regulation policies. Oversee utilization management processes by evaluating diagnostic evaluations and coordinating appropriate services for inpatient, outpatient, or hospice care settings. Requirements Valid nursing license with current registration; experience as a registered nurse in acute care settings such as ICU, Emergency Room, or Level I Trauma Center preferred. Demonstrated critical care experience including Level II trauma center or Level I Trauma Center experience is highly desirable. Proven background in hospital medicine, medical-surgical units, or home health nursing with familiarity in managing complex cases involving geriatrics or pediatrics. Strong knowledge of physiology, medical terminology, infection control practices, and health regulation policies. Experience with EMR/EHR systems for documentation and case tracking; proficiency in managing electronic health records is essential. Excellent patient service skills with the ability to communicate effectively across diverse populations; experience in client communication and discharge planning is advantageous. Ability to work collaboratively within multidisciplinary teams while managing multiple priorities efficiently in inpatient or outpatient environments. $85,000.00 - $91,000.00 per year
