This position is responsible for providing specialized wound and skin care services while supporting the overall health and well-being of residents in a post-acute and long-term care environment. Key Responsibilities Provide wound and skin care assessments, treatment planning, and ongoing monitoring for residents. Implement preventative measures for residents at risk of skin breakdown and pressure injuries. Collaborate with physicians, nursing staff, and interdisciplinary teams to coordinate individualized care plans. Administer medications and treatments as ordered and document care accurately and timely. Monitor residents for changes in condition and communicate updates appropriately. Support and guide nursing assistants and clinical staff in daily care activities and best practices. Participate in infection prevention initiatives and maintain compliance with healthcare regulations and standards. Assist with admissions, transfers, and discharges as needed. Contribute to staff education, orientation, and clinical training programs. Maintain accurate nursing documentation, reports, and resident records. Perform additional duties as assigned to support resident care operations. Qualifications Active Registered Nurse (RN) license in the State of Michigan. Graduate of an accredited nursing program. Current CPR certification required. Previous wound care experience preferred. Experience in long-term care, skilled nursing, or rehabilitation settings preferred. Strong clinical judgment, organizational skills, and attention to detail. Ability to manage multiple priorities in a fast-paced environment. Knowledge of infection control practices and nursing regulations. Basic computer proficiency and electronic documentation skills required. Physical Requirements Ability to walk, bend, lift, reach, push, pull, and sit for extended periods as needed. Must be able to perform essential job duties with or without reasonable accommodation. Commitment to maintaining workplace safety and infection control standards.
