Schedule:

Monday to Friday, 8:00 AM 5:00 PM Full-time, 13-week assignment Caseload expectation: 30 points weekly (typically 35 patient visits per day) Daily travel within assigned territory (mileage reimbursement provided)

Qualifications:

Active RN license in good standing for the state Minimum of 1 year of recent Home Health experience, with field-based practice preferred Proficiency with Home Care Home Base (HCHB) electronic documentation system Solid experience conducting OASIS assessments Valid drivers license, reliable transportation, and auto insurance

Primary Responsibilities:

Conduct comprehensive OASIS assessments, developing personalized care plans for home health patients Deliver skilled nursing, including wound care, medication administration, IV therapy, and management of chronic conditions Monitor and evaluate patient progress, identifying any changes in condition and communicating effectively with the broader care team and physicians Educate patients and families on health management, treatment protocols, medications, and preventive health strategies Maintain timely, accurate records in HCHB Collaborate closely with physical therapists, occupational therapists, social workers, and home health aides to provide coordinated, holistic care Participate in case conferences, staff meetings, and continuing education
