Responsibilities:

Conduct initial client assessments and continuously refine care plans. Provide hands-on nursing care and patient education. Qualifications:

Licensed as a Registered Nurse / RN in the state you work in. Proficiency with Electronic Medical Records (EMR) software. Excellent planning, organizing, and communication skills.
