Valid RN State Licensing required MN State Licensing preferred. CPR certification and maintenance, Valid MN Driver's License, current insurance or be insurable.
Summary:
Responsible for providing medical cares to our community members in their homes, while also working as a liaison in accessing other services our clients may need.
Education Requirements and Experience:
Current RN, with at least two years of experience in Long Term Care facility, Primary Care or Home Care.
Additional Qualifications and Skills:
Excellent time management, organization, conflict resolution, public relations and speaking skills are required. Ability to establish and maintain effective working relationship with staff, clients, and other professionals. Ability to effectively and efficiently communicate, both verbally and in writing is required. All licensing renewal requirements as specified by the MN state Board of Nursing or as required by the state they are licensed in, including required CEU's. Familiarity with the Leech Lake Reservation and surrounding communities is preferred. Experience in utilization of Electronic Health Record (EHR) technology or willingness to learn. Native American preference. Must meet Health Division hiring guidelines. Pre-employment drug testing is mandatory for all Leech Lake Band of Ojibwe positions.

None.
Duties/Responsibilities:
Visits clients in their homes, spends 75% of the position in homes. Provide individualized nursing care to patients. Collaborates with other professionals to plan, implement and evaluate care. Setting up medications, and evaluating client's compliance. Demonstrate competency and use of nursing process (assessment, diagnosis, outcomes/planning, implementation and evaluation). Utilizes appropriate age and population standards. Cares for patients in all phases of preventative care, health maintenance, diagnosis, treatment, and follow up as patients move along the continuum of care. Responsible for planning and coordination of care, patient assessment, patient education, triage, and various other nursing interventions. Wound care. International Normalized Ratio (NR), and blood draws in the home. International Normalized Ratio (INR), and blood draws in the home. Vaccinate in the home. Appropriate, accurate and complete medical record documentation. Functions within the scope and standards of nursing practice as outlined in the Nurse Practice Act and Administrative Rules in state of practice and licensure. Utilizes Fleet or Tribal vehicles daily, personal vehicles only allowed with manager or director of nursing approval. Attend and or conduct all mandatory meetings and trainings as required. Maintain confidentiality and professional ethics at all times. Travel in and out of the state on a possibly regular basis. Understanding and sensitivity to Native American culture. Adhere to policies governing staff conduct. All other related duties as assigned.
Physical Requirements:
May be exposed to communicable diseases and/or blood borne pathogens including Coronavirus. May require driving in sub-optimal weather and road conditions. Subject to internal working conditions. May require occasional modifications of workstations (this position will need to provide services in a large variety of settings and community locations). Lifting requirement of at least 251bs minimum. Non-smoking environment at workplace, subject to second hand smoke in homes.
