MDS Coordinator -
Skilled Nursing Facility Location:
Saint Charles, MN Job Type:
Full-Time A skilled nursing facility in Saint Charles, Minnesota is seeking an experienced MDS Coordinator to join our clinical team. The MDS Coordinator will be responsible for coordinating and completing the MDS assessment process, supporting accurate resident care planning, and helping ensure compliance with applicable Medicare, Medicaid, and regulatory requirements. Responsibilities Coordinate and complete MDS assessments accurately and within required timelines Manage the RAI/MDS process from assessment through completion Coordinate with nursing, therapy, social services, dietary, and other interdisciplinary team members Participate in resident care planning and care plan meetings Monitor assessment schedules and ensure timely completion of required documentation Review clinical documentation for accuracy and completeness Support appropriate reimbursement and case-mix management Maintain compliance with CMS and state requirements Educate and support clinical staff regarding MDS documentation and processes Assist with audits, surveys, and other regulatory reviews as needed Qualifications Current nursing license in Minnesota; RN preferred Previous MDS experience in a skilled nursing or long-term care setting strongly preferred Working knowledge and experience with PointClickCare (PCC) required Knowledge of MDS, RAI, Medicare, Medicaid, and skilled nursing reimbursement Strong organizational and time-management skills Excellent attention to detail Ability to work collaboratively with an interdisciplinary team Experience working with electronic medical records and clinical documentation systems What We Offer Full-time opportunity Supportive clinical and administrative team Professional and collaborative work environment Opportunities for continued growth and development Competitive compensation and benefits If you are an experienced MDS professional looking to join a skilled nursing team where your knowledge and attention to detail can make an impact, we encourage you to apply. $38.00 - $48.00 per hour
