Deliver patient-centered care as an experienced Registered Nurse specializing in home health, supporting individuals within their own residences. This full-time, 13-week contract role is field-based, with nursing visits conducted in patients homes and a focus on clinical excellence.

Schedule:

Monday Friday, 8:00 AM 5:00 PM Responsibilities include: Performing comprehensive OASIS assessments and formulating personalized care plans for each patient. Delivering skilled nursing services such as wound care, administering medications, IV therapy, and managing chronic illnesses. Monitoring patient condition, evaluating progress, and communicating status changes to the wider care team and physicians. Providing educational support to patients and their families on disease processes, prescribed treatments, medications, and preventive strategies. Maintaining thorough and prompt documentation in the Home Care Home Base (HCHB) system. Collaborating effectively with physical and occupational therapists, medical social workers, and home health aides to coordinate well-rounded patient care. Participating in routine case conferences, staff meetings, and professional development sessions. Ensuring compliance with regulatory requirements and internal agency standards in every aspect of care.

Required qualifications:

Valid, active RN license in the state. Minimum one year of recent home health nursing experience, with field-based expertise strongly preferred. Proficiency with Home Care Home Base (HCHB) electronic documentation. Demonstrated experience with OASIS assessments. Dependable transportation, current drivers license, and auto insurance required.
