Embark on a dynamic role providing in-home nursing care with a full-time, 13-week contract for a dedicated Registered Nurse specializing in Home Health. This field-based position involves serving patients in their homes, ensuring quality care and continuity in a rewarding, autonomous setting.

Schedule:

Full-time, 13-week contract Monday Friday, 8:00 AM 5:00

PM Role Expectations:

Complete 30 points per week (35 patient visits daily) Conduct daily travel within assigned territory (mileage reimbursement provided)

Key Responsibilities:

Conduct comprehensive OASIS assessments and develop individualized patient care plans Provide skilled nursing services, including wound care, IV therapy, medication management, and chronic disease interventions Monitor and evaluate clinical progress, promptly communicating changes to the interdisciplinary team and physicians Educate patients and families on disease processes, medications, treatment plans, and preventive care strategies Maintain thorough, timely documentation using Home Care Home Base (HCHB) Collaborate effectively with therapists, social workers, and home health aides to facilitate comprehensive patient care Participate in case conferences, staff meetings, and ongoing education Ensure adherence to regulatory and agency standards for documentation and care delivery

Qualifications:

Active RN license in good standing for the state of practice At least 1 year of recent field-based Home Health nursing experience Proficiency with Home Care Home Base (HCHB) and OASIS documentation is essential Reliable transportation, valid drivers license, and current auto insurance
