Job Summary We are seeking a dedicated and detail-oriented Home Health RN Admissions Nurse to join our team. The ideal candidate will possess extensive clinical experience, including critical care and demonstrate a strong ability to assess patient needs accurately. As an integral part of our home health services, the Admissions Nurse will coordinate patient intake processes, evaluate clinical conditions, and ensure seamless transitions from hospital or other settings into home health. This role offers an opportunity to make a meaningful difference in patients' lives by providing expert nursing assessments and facilitating personalized care plans. Qualifications 1. Registered Nurse licensed with State of Minnesota 2. At least two years Medical/Surgical or related nursing experience 3. OASIS experience preferred 4. Knowledge of state/federal home care regulations and licensure rules preferred Accountable To Home Health Clinical Manager Primary Objective Provide initial/comprehensive nursing assessment, skilled nursing care, and establish/develop the plan of care to complete the admission process for home care patients, in his/her place of residence. Knowledge and Skills Required 1. Knowledge of professional nursing practices and principles 2. Excellent physical and psychosocial assessment skills 3. Knowledge in developing nursing care plans according to assessments and patient needs 4. Knowledge of current methods to manage acute/chronic diseases, medication regimen, symptoms, pain control. 5. Ability to adapt nursing principles and practices to a variety of home settings 6. Knowledge of current methods in acute and chronic illness, geriatric nursing, terminal illness. 7. Mobile device / computer knowledgeable with keyboarding skills necessary to perform clinical charting within an electronic medical record 11. Essential Duties 1. Attendance for reliability for clients to receive uninterrupted services 2. Completes the initial/comprehensive nursing assessment to determine patient needs, develops/executes plan of care interventions for each individual patient that lead to positive outcomes, updates physician/plan of care as needed. 3. Provides skilled nursing care under physician orders. 4. Promotes, maintains, and restores health through teaching, counseling, and use of appropriate preventative, rehabilitative and /or palliative measures, and with ongoing observation and assessment of response. 5. Collaborates with the patient's physician regarding the plan of care and arranges/assures the Face to Face Encounter with PCP is completed as required. 6. Assures quality of care is maintained and client's needs are appropriately met. 7. May carry a small caseload of patients 8. Maintains and completes accurate documentation in timely manner. a. Point of care documentation will be completed with the expectation that at least 80% of the documentation is done in the patient's home. b. Admission/Recertification/Resumption of Care evaluation visits should be completed and synced into database within 24 hours. c. Transfer/Discharge visits should be completed and synced in on the same day. d. Subsequent visits should be completed and synced in on the same day. 9. Participates in case conferences and/or IDT meetings 10. Meets established productivity standards 11. Schedules/completes visits during regular business hours and per weekend/holiday rotation schedule. 12. Attends staff meetings and educational programs as required. 13. Follows State and Federal Regulations for Home Care and Hospice 14. Follows all safety policies and procedures. 15. Delegates personal care or homemaking duties to home health aides and homemakers. Participates in instruction, supervision and evaluation of aides and homemakers 16. Assists with Quality Assurance Performance Improvement (QAPI) and Process Improvement Projects (PIP) as requested 17. Other duties as assigned Reliable Transportation Necessary Work Environment 1. Requires daily travel by automobile in all weather conditions 2. Occasional exposure to unclean or unsanitary conditions during course of home visits including cats, dogs, smokers. Physical Demands This position involves sitting, driving, standing, walking, repeated bending, squatting, stooping or similar strenuous duties, hand repetitive movements and viewing a digital monitor. This position also involves lifting to 50 lbs. occasionally.
Pay:
$43.00 - $48.00 per hour
