Home Health RN Quality Specialist:

Position Summary:
Requires timely and appropriate care interventions and interdisciplinary communication to enhance patient satisfaction. Ensures quality documentation and outcomes. What's in it for me?

Otsego Office and Remote Hours:
8-10 hours/week with opportunity for growth Competitive hourly rate $34.00- $38.00/ hour

K, CEUs Essential Job Duties and Responsibilities:
Demonstrate critical thinking to advocate for patient safety, adherence to company policies, and conflict resolution. Review data entry in medical record system to ensure accuracy for billing and compliance. Conduct patient admission assessments, care planning, goal setting, evaluation, coordination of care, emergency/safety plans as needed per manager direction. Ensure the development and implementation of the individualized plan of care. Assist with HHCAHPS survey analysis and implementation of quality improvement activities. Understand and comply with policy and procedures set forth by AHC. Monitors operations to ensure compliance with county, state and federal regulations affecting home health care. Additional tasks, as assigned by manager.
Education Requirements:
Must be licensed as a Registered Nurse (RN) in Minnesota.
Additional Requirements:
Working knowledge of Microsoft Office suite. Excellent oral and written communication skills. Ability to read, write and speak fluently in English. Education and experience in the scope of services offered by the agency. Two years of home care experience including OASIS data collection, preferred. Maintain confidentiality in accordance with applicable laws and rules. Provide four week written notice prior to termination of employment.
