Job Description

Overview We are seeking a dedicated and highly skilled Licensed Practical Nurses and Registered Nurses to join our healthcare team for the PM (afternoon/evening) shift, 2:30pm-11pm. The ideal candidate will possess a comprehensive understanding of patient care, demonstrate exceptional clinical skills, and be committed to providing compassionate, high-quality nursing services. This role offers an opportunity to work in a dynamic environment where patient safety, comfort, and well-being are prioritized. The Nurse, PM Shift, will collaborate with multidisciplinary teams to deliver efficient and effective care across various medical settings. Duties Perform thorough patient assessments, including vital signs, physical examinations, and health history documentation using electronic health record (EHR) systems such as Point Click Care. Administer medications and injections accurately following HIPAA guidelines and aseptic techniques. Monitor patients' conditions closely, utilizing critical care experience when necessary. Provide wound care, suturing, ostomy management, and tube feeding while maintaining sterile processing standards. Support patients with complex needs including dementia or Alzheimer's care, memory care units, or behavioral health requirements. Collaborate with physicians and specialists or infectious disease care to ensure optimal patient outcomes. Educate patients and families on health coaching topics like pain management, discharge planning, or medication administration. Requirements Valid nursing license with current registration in the relevant jurisdiction; BLS (Basic Life Support) certification required. Proven experience in skilled care environments such as skilled nursing home, acute medicine, emergency room or rehabilitation centers. Experience in geriatrics, long-term care facilities, assisted living communities, or home & community care settings. Competence in advanced nursing procedures such as IV insertion and infusion therapy. Knowledge of infection control protocols and compliance with NCQA standards is essential. Excellent communication skills coupled with a compassionate approach to patient assessment and care delivery. Pay:
$27.00 - $47.00 per hour
