Great Nursing Shouldn't Depend on How Close Someone Lives to a Hospital. Not every patient lives five minutes from a major medical center. Some are recovering after hospitalization in Princeton. Others are managing chronic conditions in Cambridge, Isanti, Zimmerman, or East Bethel. Some have mobility limitations, transportation challenges, or simply need skilled nursing where they're most comfortable. At home. Home Health Care, Inc. is hiring a Registered Nurse (RN) Visit Nurse to bring skilled nursing directly to patients throughout the northern Metro and surrounding communities. You'll help extend high-quality clinical care beyond hospitals and clinics and into the places where patients are actually recovering. Bring the Care to Them For many patients, getting to another appointment isn't simple. They're weak after hospitalization. Driving may not be possible. Getting in and out of a vehicle can be difficult. A spouse or family member may already be juggling medications, appointments, meals, and everyday care. That's where home health nursing matters. You'll walk through the patient's front door ready to assess what's happening, provide the skilled treatment they need, educate the patient and caregiver, recognize changes, and communicate concerns back to the clinical team. The same clinical judgment you developed in skilled nursing matters when a patient's condition changes at home. The same wound care, medication management, disease education, and communication skills you've spent years developing are still needed. The setting is different. The importance of the nurse isn't. As a Visit Nurse, you'll take those skills throughout the community and provide the hands-on nursing patients need to safely continue their recovery. What You'll Do Provide skilled nursing visits throughout your assigned territory Complete nursing assessments and ongoing evaluations Perform wound care and other ordered skilled treatments Provide medication management and education Monitor chronic and medically complex conditions Support patients following hospitalization or changes in condition Recognize clinical concerns and communicate them appropriately Educate patients and caregivers Support admissions and additional skilled visits as needed Collaborate with RN Case Managers, physicians, therapists, social workers, and other clinicians Maintain accurate and timely clinical documentation Participate in required on-call and weekend rotations A Different Kind of Community Nursing Your patients may live in Andover, Ham Lake, St. Francis, Oak Grove, East Bethel, Isanti, Cambridge, Zimmerman, Princeton, Clear Lake, Braham, Stanchfield, and surrounding communities. They may have very different diagnoses, backgrounds, and care needs. What they have in common is that they need a skilled nurse willing to come to them. Active Minnesota Registered Nurse (RN) license Strong clinical assessment and critical-thinking skills Confidence providing skilled nursing care in patient homes Excellent patient and caregiver education skills Strong organization and communication Valid driver's license and reliable transportation Ability to travel throughout the assigned service areaPreferred:
Home health experience Hospice experience TCU or post-acute experience Experience providing skilled nursing independently in the field Take Great Nursing Beyond the Hospital Walls.
