This role is ideal for experienced nurses who want independence in the field while still having strong clinical support behind them. Floating case managers play an important role in keeping care running smoothly across our service areas. You will help cover growing caseloads, assist during staffing transitions, and provide high-quality nursing care to patients in different communities. In this role, you will complete skilled nursing visits, perform assessments, and manage patient care plans while working across multiple territories as needed. You will help ensure continuity of care, support team members, and maintain high clinical standards throughout our service area. Floating RN Case Manager Responsibilities Provide skilled nursing visits and patient assessments in the home. Develop and update individualized plans of care. Educate patients and caregivers on medications, treatments, and safety. Support different territories based on census and staffing needs. Assist with admissions, routine visits, and case management coverage. Coordinate care with therapists, physicians, and other clinicians. Complete accurate documentation including OASIS when required. Participate in rotating on-call coverage. RN Case Manager Qualifications Active Minnesota Registered Nurse license. Valid driver's license and reliable transportation. Ability to work independently in a field-based setting. Strong assessment and care coordination skills. Home health experience preferred but not required.
