A patient can be medically ready to leave the hospital or rehabilitation facility and still have a lot of questions about what happens once they walk through their front door. New medications, mobility concerns, wounds, equipment, caregiver responsibilities, follow-up appointments, and new diagnoses can make those first few days at home overwhelming. That's where you come in. Home Health Care, Inc. is hiring experienced Weekend Start of Care RNs throughout Southwestern Minnesota to help patients successfully transition into home health. You'll earn $200 per Start of Care visit plus mileage reimbursement , with opportunities to work every weekend or every other weekend . Help Make the Transition Home Actually Work As the Start of Care Nurse, you're one of the first clinicians to see what recovery looks like outside the controlled environment of a hospital or facility. You'll compare the discharge plan to what's actually happening at home, complete a comprehensive assessment, reconcile medications, identify immediate needs, educate the patient and caregiver, and establish the initial home health plan of care. For an experienced home health RN, it's an opportunity to use your assessment skills, OASIS knowledge, and clinical judgment during one of the most important points in a patient's recovery. They return home on Saturdays. Their medications, wounds, chronic conditions, and safety concerns don't wait for the traditional workweek to begin. Weekend admissions allow us to get into the home sooner, understand what the patient needs, and begin coordinating appropriate services. Your job is to help close that gap between leaving one level of care and successfully beginning the next. What You'll Be Doing Complete comprehensive Start of Care nursing assessments Perform accurate and timely OASIS documentation Complete medication reconciliation and identify discrepancies or concerns Assess clinical, functional, environmental, and safety needs Develop individualized plans of care Educate patients and caregivers on medications, diagnoses, safety, and home health expectations Identify concerns requiring physician or provider communication Coordinate with referring providers and physicians Collaborate with RN Case Managers, therapists, and other members of the interdisciplinary team Provide thorough documentation and communication for the clinicians continuing the patient's care We're Looking for Home Health Experience Start of Care nursing requires an RN who can independently enter a new environment, gather a significant amount of information, determine what's clinically important, and document it accurately. OASIS and SOC experience are important for this position , because we want you to be comfortable taking ownership of the admission from the beginning. This could be an especially strong fit for a current or former Home Health RN Case Manager, Admissions RN, SOC Nurse, or experienced field RN looking to put those skills to work on weekends.
What You Bring Required:
Active Minnesota Registered Nurse (RN) license Previous home health nursing experience Experience completing Start of Care assessments Strong working knowledge of OASIS Strong assessment and critical-thinking skills Accurate and timely clinical documentation Confidence working independently in patient homes Excellent patient, caregiver, and provider communication Valid driver's license and reliable transportation
Preferred:
