We want that nurse. Legacy Home Care and Home Health Care, Inc. are hiring a Floating RN Case Manager to provide coverage throughout the Greater Twin Cities Metro Area. This isn't simply an RN position without a permanent territory. It's a role for an experienced nurse who can assess quickly, prioritize confidently, communicate effectively, and take ownership of patient care wherever the need is greatest. In return, you'll have $95,000-$130,000 annual compensation, additional Incentive Visit Pay, mileage reimbursement, strong benefits, significant field independence, and an experienced clinical team behind you. Be the RN We Call When It Matters Not every nurse enjoys walking into a caseload they didn't build. The right float nurse can. You'll review the patients, understand what's happening clinically, identify who needs attention first, and make sure nothing important gets missed. You may be supporting an established territory during PTO, helping with increased census, covering an unexpected absence, or providing additional clinical capacity where the team needs it most. You know how to complete a comprehensive assessment, recognize a meaningful change in condition, determine what needs immediate attention, communicate with a physician, educate a family, coordinate with therapy, and keep the plan of care moving. That ability to walk into an existing situation and quickly understand what needs to happen next is exactly what makes an experienced nurse so valuable in this position. What You'll Do Provide skilled nursing and case management coverage throughout the Greater Twin Cities Metro Area Cover established territories during PTO, vacation, illness, leaves, and changing census needs Complete comprehensive nursing assessments Provide admissions, routine visits, and case management coverage Develop, implement, and update individualized plans of care Recognize changes in condition and respond appropriately Educate patients and caregivers on medications, safety, and disease management Coordinate with physicians, therapists, social workers, and other clinical professionals Complete accurate and timely documentation, including OASIS when applicable Participate in rotating on-call responsibilities Maintain continuity of care as territory and staffing needs change Home Health Experience Can Take You Further Here If you've already succeeded as a Home Health RN or RN Case Manager, you've developed skills that aren't easy to teach. You know how to manage patients independently, organize a field schedule, communicate across disciplines, handle changing clinical situations, and make decisions without having another clinician down the hallway. We're especially interested in RNs with backgrounds in Home Health, Hospice, Community Nursing, Case Management, Transitional Care, Acute Care, Skilled Nursing, and other field-based or complex clinical environments. Home health and OASIS experience are strongly preferred.
