This isn't simply an RN position without a permanent territory. What You'll Do Provide skilled nursing and case management coverage throughout the Greater Twin Cities Metro Area Cover established territories during PTO, vacation, illness, leaves, and changing census needs Complete comprehensive nursing assessments Provide admissions, routine visits, and case management coverage Develop, implement, and update individualized plans of care Recognize changes in condition and respond appropriately Educate patients and caregivers on medications, safety, and disease management Coordinate with physicians, therapists, social workers, and other clinical professionals Complete accurate and timely documentation, including OASIS when applicable Participate in rotating on-call responsibilities Maintain continuity of care as territory and staffing needs change Home Health Experience Can Take You Further Here If you've already succeeded as a Home Health RN or RN Case Manager, you've developed skills that aren't easy to teach. You know how to manage patients independently, organize a field schedule, communicate across disciplines, handle changing clinical situations, and make decisions without having another clinician down the hallway. We're especially interested in RNs with backgrounds in Home Health, Hospice, Community Nursing, Case Management, Transitional Care, Acute Care, Skilled Nursing, and other field-based or complex clinical environments.
