Registered Nurse Case Manager National Home HealthCare Inc - 3.5 Oakdale, MN Job Details Per diem $62 a day 1 day ago Benefits Flexible schedule Paid sick time Qualifications Electronic health records (EHR) management RN License Driver's License Full Job Description RN Case Manager - Home Health Care We are seeking a compassionate and dedicated Registered Nurse to provide high-quality care to patients in the comfort of their own homes. The ideal candidate will be responsible for managing a range of nursing duties, ensuring the delivery of individualized care plans, and fostering positive relationships with patients and their families.
Key Responsibilities:
Conduct comprehensive intake health assessments and supervisory visits to assess patient needs and develop personalized care plans. Educate patients and their families on effective healthcare techniques, disease management, and self-care practices to promote optimal health outcomes. Collaborate with healthcare professionals to ensure coordinated, holistic patient care and seamless communication. Maintain accurate and up-to-date patient medical records, documenting interactions, interventions, and patient progress in a timely manner. Monitor and report any changes in patient conditions, symptoms, or progress to the care team and make appropriate recommendations. Create a safe and comfortable home environment for patients, providing emotional support to both patients and their families.
Qualifications:
Graduate of an accredited school of nursing. Valid unencumbered RN license in the state of Minnesota. Minimum of 1 year of experience in home healthcare, hospital, or similar clinical settings. Ability to work independently, manage time effectively, and prioritize patient care needs. Proficiency in using electronic medical record (EMR) systems.

Per Diem, Flexible Schedule Salary :
$62.00 per visit
Benefits:
Paid sick time available This is a per diem position and is not eligible for benefits
Schedule:
Visits generally are scheduled between 8am - 5pm; times are coordinated between RN and the client. Flexible schedule with no weekend or evening shift requirements. Ability to

Must be a licensed driver with a reliable automobile that is insured in accordance with state and/or organization requirements. Position located in the St. Paul area.
