What You'll Be Doing Perform comprehensive assessments and develop individualized care plans Manage ongoing visits and patient education Complete OASIS assessments and documentation Coordinate care with physicians and interdisciplinary teams Support patients and families throughout the recovery process Use Your Clinical Judgment Manage chronic conditions and medically complex patients Identify changes in condition and intervene proactively Help prevent avoidable hospitalizations Serve as an advocate and trusted resource for patients and families Build meaningful relationships that make a lasting impact What You Bring Required Active Minnesota RN license Three or more years of nursing experience Strong assessment and communication skills Reliable transportation and valid driver's license Preferred Home health experience OASIS experience Previous case management experience Some Nurses Count Tasks.
