Develop care plans including discharge planning, coordinate delivery among providers, and teach patients and families how to follow the plan. They may also help patients navigate insurance and other healthcare bureaucracies and access related social services. May work with patients who have chronic illnesses or other complex medical needs. Works at a hospital or other healthcare facility; may follow patients throughout the span of their treatment.

- Part-time The Mary T Family of Companies has an immediate opening for a part-time Home Health RN Case Manager to serve clients in the areas of Rush City, Braham, and Pine City, MN.
Responsibilities include:
Accepts home health referrals and completes initial assessment Regularly evaluates the care needs of the patient and make necessary revisions to the patient's plan of care Makes referrals to other disciplines as indicated by the patient's needs, or documents rationale for not doing so Teaches, supervises, and counsels the patient and patient's family regarding home health procedures, interventions and other care needs as appropriate to the patient's condition Supervises and teaches the home health aides about their responsibilities in meeting the patient's care plan needs Ensures that the documentation required for reimbursement of care is completed on a timely basis, and in compliance with State and Federal regulations Performs duties necessary to maintain quality standards Consistently facilitates positive interpersonal relations among staff Ensures service satisfaction through patient visits and communication with other patient nurses and caregivers On-call triage on a rotating basis for all home care patients Facilitates patient and caregiver independence to the extent possibleSchedule:
Daytime Hours 8:30 AM- 4:30 PM , part-time hours available, up to 24 hours per week.
Caseload:
This position will have a maximum caseload of 25 patients, with LPN support.Requirements:
