Maybe a patient isn't taking a medication because they don't understand its purpose. Maybe a caregiver isn't comfortable with wound care. Maybe someone with CHF doesn't understand why daily weights matter. Maybe a diabetic patient nods through the education but still doesn't feel confident managing their condition. What You'll Be Doing Provide skilled nursing visits throughout your assigned territory Complete nursing assessments and ongoing evaluations Perform wound care and other ordered skilled treatments Provide medication management and education Teach patients and caregivers about diagnoses and treatment plans Reinforce discharge instructions and recovery plans Monitor chronic and medically complex conditions Recognize changes in condition and respond appropriately Support patients following hospitalization, surgery, illness, or injury Complete accurate and timely clinical documentation Communicate findings and concerns to RN Case Managers and physicians Collaborate with PTs, OTs, SLPs, social workers, and other clinicians Participate in an on-call and weekend rotation Bring Your Clinical Experience Into Their World We're interested in RNs from Home Health, Med/Surg, Acute Care, TCU, Skilled Nursing, Rehabilitation, Hospice, Post-Acute Care, and other hands-on nursing environments . What You Bring Required:
