The RN will perform assessments, provide evaluation and create a plan of care in coordination with the Rheumatologists. Other duties will include infusion therapy, triage, clinical care coordination with the healthcare team. This is a part-time (casual; no set schedule) position working as needed Monday-Friday 7am-7pm for PTO and coverage needs out of our Woodbury location. Twin Cities Orthopedics is an operating division of Infinite Health Collaborative (i-Health), a group of local medical practices, owned and led by its physicians, on a mission to give patients more control over their healthcare.
Essential Functions:
Nursing Functions:
Follows and implements designated policy and procedures. Demonstrates clinical competency, and a holistic approach to patient care, caring for the whole patient.
Holistic Care Coordination:
Caring for the whole patient to ensure support of the family and patient throughout the care continuum. This includes assisting the patient and family navigation of the care system in order to provide optimal care to meet the needs of the patient. Collaborates with the healthcare team to ensure optimal patient outcomes and results. Ability to establish rapport with the patient to work on shared goals and priorities.
Documentation:
Ensuring accurate clinical care is documented in the EMR.
Collaboration:
Collaborates with the care team around essential operational functions such as patient referrals, clinical care coordination with the care team and physicians to include out-patient settings. Other duties as assigned
Education and Experience Requirements:
Graduate of an Accredited School of Nursing Current RN licensure in Minnesota or Wisconsin - depending on clinic locations Current BLS certification for health professionals through the American Heart Association or Red Cross required and maintained.

Working Conditions:
Frequently lift up to 50 pounds independently and occasionally up to 100 pounds with assistance. Requires the ability to stand, walk, bend, and move continuously throughout the shift. Must be able to frequently perform physical activities such as pushing, pulling, and reaching. Visual acuity to read electronic and paper documents. Auditory ability to participate in phone or video calls clearly. Manual dexterity to operate standard office equipment (e.g., computer, phone, printer).
