Home health risk assessments typically last about 45 minutes and may include annual health assessments or post-discharge evaluations.

Key Responsibilities:

Conduct annual home health risk assessments for Medicare Advantage plan members Evaluate medical history, chronic conditions, and medication use Offer education regarding ongoing health concerns and prescribed medications Identify gaps in care and recommend appropriate next steps, as necessary Focus on assessmentno treatment planning or prescribing required

Qualifications:

Active Nurse Practitioner license in the state of Missouri Current CPR (BLS) certification Board Certified through AANP or ANCC Six months or more of recent NP experience preferred MSN from an accredited nursing program Compensation for this role includes $80$90 per evaluation, as well as a mileage stipend and additional pay for administrative time.
