13-week contract with possible extension Full-time, Monday through Friday On-call responsibilities may apply

Essential Qualifications:

Active RN license At least one year of recent home health experience (required) Strong OASIS proficiency required for timely and accurate documentation Experience with HomeCare HomeBase EMR (strongly preferred) Ability to manage a structured daily visit schedule Reliable transportation and a valid drivers license

Primary Responsibilities:

Deliver skilled nursing care and individualized interventions in patients homes Perform comprehensive patient assessments and document findings using OASIS Collaborate with physicians and interdisciplinary teams to construct and manage tailored care plans Provide wound care, manage medications, and educate patients and families on disease processes and self-care Monitor patients conditions and provide timely updates to the clinical team
