Key Responsibilities:

Conduct comprehensive annual home health risk assessments for Medicare Advantage members in their residences Review prior and current medical/medication history and ongoing health conditions Educate patients on their medical conditions and medications, supporting their understanding and engagement in care Identify gaps in care and provide recommendations for care coordinationprescribing or creating treatment plans is not required Assignments will be scheduled in any one of several communities within Callaway County during each shift

Qualifications:

Active Nurse Practitioner license in Missouri Current CPR certification (BLS) Board Certification through AANP or ANCC Minimum six months of current NP experience preferred MSN degree from an accredited school of nursing This per diem opportunity offers compensation of $80-$90 per in-home evaluation, plus a mileage stipend and additional pay for administrative time.
