Key Responsibilities:

Perform annual home health risk assessments within patients residences Assess current and prior medical history, medical conditions, and review medication usage Educate members regarding their health conditions and prescribed medications Identify care gaps and make recommendations to support better health outcomes Visits may vary in nature, including annual assessments or post-discharge follow-ups No responsibility for developing treatment plans or prescribing medications

Required Qualifications:

Active Nurse Practitioner license in Missouri Current CPR (BLS) certification Board certified through AANP or ANCC At least 6 months of current experience as a Nurse Practitioner preferred Masters degree (MSN) from an accredited nursing program
