$2,360.00 - $2,580.00
Productivity Expectations:
25 productivity points per week Manage a caseload of approximately 12-14 patients Assist with hospice admissions as scheduling allows
Required Skills & Qualifications:
Active RN license required. Previous hospice case management experience required. Experience with Homecare Homebase (HCHB) required. Strong clinical assessment, care coordination, and documentation skills. Ability to travel throughout the assigned service area. Comfortable participating in on-call and weekend rotation.
Additional Information:
Professional scrubs (no specific color required).
Coverage Area:
Seven-county radius, primarily serving the Hannibal and northern territories, with travel to the office at least once per week
