Key Responsibilities:

Conduct comprehensive home health risk assessments, lasting approximately 45 minutes each Review and evaluate patients prior and current medical history, including chronic conditions and medications Deliver clear education to participants about their health and treatments Recognize care gaps and offer expert, actionable recommendations for ongoing care Focus solely on evaluationno treatment planning or medication prescribing is required

Qualifications:

Active Nurse Practitioner license in MO Board certification via AANP or ANCC Current CPR (BLS) certification Masters in Nursing (MSN) from an accredited institution At least 6 months of recent Nurse Practitioner experience preferred
