Schedule:

Full-time, Monday through Friday On-call responsibilities may be required

Key Responsibilities:

Deliver skilled in-home nursing care and interventions for a diverse patient population Complete patient assessments and accurate OASIS documentation in a timely manner Collaborate with physicians and multidisciplinary care teams to develop and follow individualized care plans Administer wound care, manage medications, conduct patient and caregiver education, and monitor ongoing disease processes Communicate promptly with the clinical team about patient condition updates or changes

Qualifications:

Active RN license Minimum 1 year of recent home health RN experience Proven OASIS assessment and documentation skills Proficiency with HomeCare HomeBase (strongly preferred) Ability to effectively manage full-time schedules with daily visit targets Reliable transportation and valid drivers license
