Key Responsibilities:

Conduct comprehensive annual home health risk assessments in patients homes Review and evaluate prior/current medical conditions, personal and family medical history, and medication usage Educate patients about their medical conditions and medications Identify gaps in care and make actionable recommendations to improve patient outcomes Focus exclusively on assessmentno treatment planning or prescribing required

Qualifications:

Active Nurse Practitioner license in Missouri Board Certification by AANP or ANCC Current CPR (BLS) certification Graduate of an accredited MSN (Master of Science in Nursing) program At least 6 months of recent NP experience preferred This role offers compensation of $80$90 per evaluation, along with a mileage stipend and paid administrative time.
