Enjoy the flexibility and meaningful impact of per diem work as a Nurse Practitioner specializing in home health risk assessments. This position offers you the autonomy to craft your own schedule with shifts available seven days a week. You'll support Medicare Advantage plan members in Saline County by providing in-person risk assessments in their homeseach visit tailored to the members needs, from annual wellness exams to post-discharge check-ins. Locations may include Marshall, Slater, Sweet Springs, and surrounding areas.

Schedule:

Flexible shifts between Monday and Sunday Self-scheduled shifts to fit your availability Minimum hourly commitment per week or bi-weekly

Key Responsibilities:

Perform comprehensive annual risk assessments for Medicare Advantage members in a home setting Assess and document past and current medical history, ongoing conditions, and medication usage Offer clear education regarding medical conditions and prescribed medications Identify care gaps and recommend appropriate next steps Focus exclusively on assessments with no responsibility for prescribing or care plan creation

Qualifications:

Active Nurse Practitioner license in Missouri Board Certification (AANP or ANCC) MSN from an accredited nursing program Current CPR (BLS) certification At least 6 months of recent NP experience preferred
