Key Responsibilities:

Perform comprehensive annual health risk assessments for Medicare Advantage members in home settings Evaluate patient medical history, current conditions, and medication regimen Educate patients on diagnoses, treatments, and medication usage Identify care gaps and recommend strategies for resolution Complete assessments, but treatment planning and medication prescribing are not required

Qualifications:

Active Nurse Practitioner license in Missouri (MO) Current CPR certification (BLS required) Board certification via AANP or ANCC At least 6 months of current NP experience preferred Master of Science in Nursing from an accredited nursing program This role offers compensation between $80 and $90 per completed evaluation, plus a mileage stipend and payment for administrative time.
