Full Job Description Role:

In-Home Nurse Practitioner Responsibilities:

Perform comprehensive health evaluations for adults and seniors in their homes. Conduct detailed assessments, review medications, and manage chronic conditions. Identify gaps in care and utilize mobile technology and data-driven tools. Collaborate with interdisciplinary teams to ensure coordinated care. Document findings in the electronic health record (EHR) system. Support value-based care initiatives and follow-up care planning.

Qualifications & Skills:

Active Nurse Practitioner license. Strong assessment and clinical management skills. Experience with chronic disease management and medication review. Proficiency with mobile technology and data documentation tools. Excellent communication and teamwork abilities.
