Key Responsibilities:

Complete annual risk assessments for Medicare Advantage members in their residences Conduct thorough reviews of medical histories, ongoing conditions, and medication regimens Offer patient education on diagnoses and prescribed medications Evaluate for any care gaps and make proactive recommendations No requirement for developing treatment plans or prescribing medications

Qualifications:

Current Nurse Practitioner license in the state of Missouri Active CPR (BLS) certification Board certification via AANP or ANCC At least 6 months of recent Nurse Practitioner experience, preferably in home health Master of Science in Nursing from an accredited institution This per diem opportunity provides attractive compensation of $80$90 per evaluation, in addition to a mileage stipend and compensation for administrative time.
