This position involves conducting home health risk assessments for Medicare Advantage plan members throughout Benton County, including areas such as Warsaw, Cole Camp, Lincoln, and White Branch. Each risk assessment takes about 45 minutes and may be focused on an annual health check or follow-up after hospital discharge. Key Responsibilities:

Perform thorough home health risk assessments for Medicare Advantage members Review prior and current medical histories, conditions, and medication regimens Educate patients on their medical conditions and proper medication use Identify gaps in care and suggest actionable recommendations No treatment planning or medication prescribing required

Qualifications:

Active Nurse Practitioner license in Missouri Board certification via AANP or ANCC Current CPR certification (BLS required) MSN degree from an accredited nursing school At least 6 months of recent NP experience preferred This per-diem opportunity offers $80-$90 per evaluation, with additional mileage and administrative time compensation.
