Seeking an experienced Nurse Practitioner to perform home health risk assessments for Medicare Advantage plan members across Benton County, MO, including Warsaw, Cole Camp, Lincoln, and White Branch. This per diem role offers the flexibility to design your own schedule with options for minimum weekly or bi-weekly hourly commitments. Assessments typically last about 45 minutes and may include annual health assessments or post-discharge visits.

Responsibilities:

Conduct in-home annual risk assessments for Medicare Advantage members Evaluate medical histories, current health conditions, and medication usage Educate patients about their health conditions and medication regimens Identify gaps in care and recommend solutions for better outcomes No treatment planning or medication prescribing required

Qualifications:

Active Nurse Practitioner license in Missouri Current CPR certification (BLS) Board Certification via AANP or ANCC Minimum 6 months of current Nurse Practitioner experience preferred MSN degree from an accredited nursing school This position compensates $80-$90 per completed evaluation.
