This position offers the flexibility to build your own schedule with shifts available any day of the week and accommodates a variety of home environments, including Boonville, Wooldridge, Blackwater, and Otterville.

Key Responsibilities:

Conduct annual and post-discharge home health risk assessments within patients residences Review and evaluate prior and current medical histories, existing medical conditions, and medication use Provide essential education to patients regarding their diagnoses and prescribed medications Identify care gaps and recommend appropriate next steps for resolution Focus exclusively on assessmentno requirement for treatment plans or medication prescribing

Qualifications:

Active Nurse Practitioner license in Missouri Board certification via AANP or ANCC Current BLS (CPR) certification MSN from an accredited nursing program At least 6 months of recent nurse practitioner experience preferred
