As part of this role, you can create your own schedule, working shifts that best fit your lifestyle, with shifts available seven days a week. Shifts may include locations such as Boonville, Wooldridge, Blackwater, Otterville, and other areas within the county.

Schedule:

Key Responsibilities:

Conduct comprehensive home health risk assessments for Medicare Advantage members Evaluate patient histories, current health conditions, and medication regimens Deliver health education regarding medical conditions and proper medication usage Identify care gaps and offer actionable recommendations No requirement to create treatment plans or prescribe medications

Qualifications:

Active Nurse Practitioner license in Missouri Board Certification through AANP or ANCC Current CPR certification (BLS) MSN degree from an accredited nursing institution At least 6 months of current experience as a Nurse Practitioner (preferred)
