Key Responsibilities:

Complete annual and post-discharge health risk assessments at patient residences throughout Bollinger County, including communities such as Glenallen, Marble Hill, Lutesville, and Zalma Engage patients in comprehensive reviews of their medical, surgical, and medication histories Offer personalized education regarding ongoing medical conditions and medication use Identify potential gaps in care and propose recommendations for clinicians and care teams No requirement for developing treatment plans or prescribing; focus is on assessment and education

Desired Qualifications:

Active Missouri Nurse Practitioner (NP) license Board Certification (AANP or ANCC) Current Basic Life Support (BLS/CPR) certification MSN degree from an accredited nursing program At least 6 months of recent NP experience is preferred
