Core Responsibilities:

Perform comprehensive home health risk assessments, including both annual and post-discharge visits Thoroughly review participants medical history, current conditions, and medications Educate patients about their health, chronic conditions, and medication management Identify care gaps and recommend actionable solutions to support improved outcomes No prescribing, treatment plan development, or ongoing case management required

Qualifications:

Active Nurse Practitioner license in Missouri Board Certification (AANP or ANCC) MSN from an accredited nursing school Current CPR (BLS) certification Minimum of six months current NP experience preferred Enjoy a competitive per-evaluation compensation structure between $80$90, plus a mileage stipend and coverage for administrative time.
