Key Responsibilities:

Conduct comprehensive home health risk assessments, averaging 45 minutes per visit, for Medicare Advantage enrollees in locations such as Mexico, Vandalia, and Laddonia. Review and document prior and current medical history, present medical conditions, and medication usage. Provide thorough education to patients on their health conditions and medication regimens during home visits. Identify care gaps and make appropriate recommendations for closing them, with a focus on improving patient outcomes. Visits may include annual wellness assessments or post-discharge check-ins; however, no prescribing or treatment plan creation is required.

Qualifications:

Active Nurse Practitioner license in the state of Missouri. Board Certification via AANP or

ANCC. MSN

degree from an accredited nursing program. Current and valid BLS (CPR) certification. At least 6 months of current experience as an NP is preferred.
