Key Responsibilities:

Conduct comprehensive annual risk assessments in patients' homes across Boonville, Wooldridge, Blackwater, Otterville, and other Cooper County communities Review and evaluate patients current and past medical history, ongoing medical conditions, and medication regimens Educate patients about their health conditions and medications, ensuring they understand their care Identify gaps in care and provide practical recommendations to improve health outcomes No requirement to develop treatment plans or prescribe medications

Qualifications:
