!! Holden Manor Health & Rehabilitation is seeking a compassionate and experienced MDS Coordinator (RN or LPN) to complete regulatory assessments for the residents in our Holden, MO location. The ideal candidate will be compassionate, cool-tempered, and will have experience in providing diverse basic care to those who need it. Must be organized and able to follow timelines for required assessments and report to the Director of Nursing. Our goal is to promote patient wellness by providing the highest quality of nursing care in the area. This role is as rewarding as it is demanding, but if you bring the passion, we'll bring the paycheck. Responsibilities Record patients' medical history accurately Coordinate & complete MDS assessments in accordance with federal & state regulations. Collaborate with interdisciplinary teams to ensure comprehensive patient care plans are developed & implemented. Maintain up-to-date knowledge of NCQA standards and regulatory changes affecting MDS processes. Facilitate discharge planning by collaborating with case management teams to ensure smooth transitions for patients. Provide training and support to nursing staff on MDS-related processes & documentation requirements & Direct staff as necessary Provide emotional & psychological support when needed Communicate with patients' family or friends to provide advice, comfort, & release instructions On-Call rotation every 3rd weekend with Nursing Administration Skills Proven experience as RN or similar role Strong knowledge of medical terminology, anatomy, physiology, & critical care practices. Experience with the MDS Assessment federal & state regulations. Ability to undertake a variety of diverse care tasks and work with a multidisciplinary team Familiarity with hospital health, safety & sanitation standards and procedures Understanding of confidentiality obligations & nursing best practices Excellent communication & interpersonal skills Compassionate & able to handle stress BLS certification preferred A true passion for caring for the elderly Holden Manor Health & Rehabilitation is part of the Beacon Healthcare Group Family of Homes.
