Responsibilities:

Complete annual home health risk assessments for Medicare Advantage members in Fulton, New Bloomfield, Tebbetts, Mokane, and surrounding Callaway County areas Review and assess patients prior and current medical history, conditions, and medication lists Educate members about their health conditions and prescribed medications Identify missed opportunities or gaps in care, making recommendations to support improved health outcomes Each assessment lasts roughly 45 minutes and may include initial or annual visits as well as post-discharge follow-ups No need to establish formal treatment plans or prescribe medications

Required Qualifications:

Active Nurse Practitioner license in Missouri Current CPR certification (BLS) National Board Certification through AANP or ANCC At least 6 months of recent Nurse Practitioner experience preferred Masters of Science in Nursing (MSN) from an accredited institution This role offers $80$90 per completed evaluation, along with a mileage stipend and compensation for administrative time.
