We are seeking a dedicated Registered Nurse (RN) with a strong background in home health and OASIS documentation for a 13-week, full-time contract assignment. Schedule and Hours:

13-week contract, full-time Monday through Friday schedule (on-call may be required) Structured daily visit expectations

Key Responsibilities:

Deliver in-home nursing care and skilled interventions to patients Complete thorough patient assessments and ensure accurate, timely OASIS documentation Collaborate with physicians and interdisciplinary teams to create and follow individualized care plans Perform wound care, medication management, patient education, and monitor chronic conditions Maintain clear communication regarding patient status and changes with the clinical team

Qualifications:

Active RN license Minimum one year of recent RN home health experience Proficiency with OASIS assessments and documentation Experience with HomeCare HomeBase (strongly preferred) Reliable transportation and a valid drivers license Capable of managing a structured, full-time schedule with multiple daily patient visits
