$85,000 - $90,000 per yearJob OverviewWe are seeking a compassionate and experienced Hospice Registered Nurse Case Manager to provide high-quality hospice care to patients in their homes and senior living settings.This is a full-time, field-based nursing role where the RN will manage an assigned hospice patient caseload, coordinate care plans, support patients and families, and work closely with the interdisciplinary care team. The ideal candidate has prior hospice experience, strong clinical judgment, and is comfortable traveling throughout the day to visit patients in their homes.

This is not a traditional shift-based role. The RN will visit multiple patients per day in their homes or senior living facilities. Most patients will be located near each other, but some visits may require a 20- to 40-minute drive.
Travel is a regular part of the role and is included in the salary, with additional mileage reimbursement provided.
