Key Responsibilities:

Complete annual home health risk assessments for Medicare Advantage members in their residences. Review medical history, current conditions, and medication use. Educate patients regarding their health status and medication regimen. Identify care gaps and recommend actions to ensure comprehensive patient care. No responsibility for prescribing medications or developing treatment plansfocus on assessment and education only.

Qualifications:

Current Nurse Practitioner license (MO) Active Board Certification (AANP or ANCC) Valid CPR certification (BLS) Minimum six months of recent NP experience preferred MSN degree from an accredited nursing program This role offers compensation of $80$90 per evaluation, plus a mileage stipend and pay for administrative tasks.
