Key Responsibilities:

Perform annual home health risk assessments tailored to Medicare Advantage health plan members Review and document comprehensive medical, medication, and surgical histories Educate patients about chronic conditions and medications, fostering improved health literacy Identify care gaps and provide actionable recommendations to enhance patient health outcomes Focus exclusively on assessmentno need to create treatment plans or prescribe medications

Qualifications:
